Eligibility Requirements
The patient resides within the service area of Hardin County Hospice.
The attending physician refers the patient to the hospice program.
The patient must have a limited life expectancy; months rather than years.
There must be a responsible person to provide care in the home.
The patient and family must want hospice care.
